Life Care Plans

In the most traditional sense, Life Care Planning is a concept that began in legal cases in which care needs and expenses became catastrophic in nature. The Life Care Plan addressed the changes, needs and expenses of an individual with long term, usually severe and chronic health care problems as a result of a personal injury of some sort. The International Academy of Life Care Planners defines the Life Care Plan as such:

“The Life Care Plan is a dynamic document based upon published standards of practice, comprehensive assessment, data analysis and research, which provides an organized, concise plan for current and future needs with associated cost for individuals who have experienced catastrophic injury or have chronic health care needs. “
